Although intrauterine visualization was first described as long ago as 1865, office hysteroscopy has not as yet been accepted by the majority of Gynaecologists. The experience of 7620 outpatient hysteroscopies demonstrates that diagnostic hysteroscopy is simple, safe and highly efficient; it is superior to any other visualizing procedure for the diagnosis of intrauterine lesions. The most important progress in hysteroscopy in recent years is the transcervical approach for treatment of intrauterine disorders that otherwise needed a transabdominal intervention. Endometrial ablation, polyp and myoma resection, dissection of cavity occlusions or uterine septa are discussed. Transcervical electrosurgery is a minimal invasive procedure, with short hospitalization time, low cost, superb patient compliance, low complication rate when safety measurements are observed. It has an excellent postoperative outcome.
